Section
This section is for all
the Facilities you see
patients at (hospitals,
surgicenters, nursing homes, etc.). Each patient is linked to a particular facility to insure correct place of service code
assignment on insurance claims.
Entering a New facility
WARNING: A least one record in this section must
be for your main office and its Code must be “OFF”. Any Code
that starts with OFF will be
considered to be an office that you see patients at.
For these select Show Address as No.
If you sell Durable Medical Equipment, or DME (which includes contact lens after
cataract surgery, believe it or not) use a different code for the
office that dispenses the DME but use the same address and select Show
Address as Yes!
Special Fields
- Name is for the name of the
facility. Do not go beyond one line or use
special characters!
- If there is a person to contact at
this facility, fill in the First, MI, Last,Title fields.
- Facility
No 1 through 4 have been replaced with specific fields for Medicare, Blue Shield, Medicaid and Tax ID Number No. The older fields
remain as a reference.
- The CLIA
No. is required if you perform laboratory tests in your office.
- For Residence there is a
pop-up list of selections if any apply to this facility.
- The field Suffix is no
longer used by has been left in for backwards compatibility.
- POS is the Place Of Service code for this facility and is a required
field. The
pop-up list shows all the accepted place of service locations with
their POS codes. The office is “11”. This list is actually stored in the PickList database. In
the event an insurance, such as Medicaid,
does not accept the standard POS codes, you can attach 2 more codes (Old
and Other) to each facility and
select in the Insurance
record which POS
code to reference on related claims. With the advent of HIPAA and ANSI
X12
transmission standard, there will be only on acceptable list in the
future.
- Show Address determines if this facility's address
gets printed
on insurance claims.
Override
Insurance Group ID
In several states, they required that the Group ID transmitted with a claim be
different depending on the location of
the office. You can enter the override for Medicare, Medicaid and Blue
Shield. So far commercial carriers do not care.
The remainder of the fields are straight forward.
